RESUMO

OBJECTIVE: The aim: This study aims in a prospective hospital-based cohort study to determine clinical and imaging features of medial medullary infarction and report a relevant clinical case in a white European adult. PATIENTS AND METHODS: Materials and methods: We have prospectively enrolled one hundred twenty adult patients with acute posterior circulation stroke. All patients were admitted and enrolled in the study within 6 to 24 hours from the onset of the stroke symptoms. Study subjects were recruited from the hospital's wards and emergency departments from 2011 to 2020. Comprehensive clinical, MRI, ultrasound, and laboratory examinations were performed on all patients. RESULTS: Results: 68 men and 52 women aged 28 to 89 years (average age 60.7 ± 12.1 years) with an acute ischemic posterior circulation stroke were enrolled in the study. Out of these 120 patients, 22 (18.3%) had acute medulla oblongata infarctions. Clinical and imaging features of medial medullary infarction are analyzed and illustrated with a clinical case presentation in a white European adult. CONCLUSION: Conclusions: Specific features of medial medullary infarction were determined, analyzed, described, and illustrated with a clinical case.

RESUMO

We have presented a brief literature overview of the disease, supported by a clinical case of multiple acute posterior circulation strokes with lesions in the pons and both hemispheres of the cerebellum associated with ovarian hyperstimulation syndrome in a white young European adult in Ukraine. Specific features of posterior circulation stroke associated with ovarian hyperstimulation syndrome were determined, analyzed, and described. Complex posterior circulation cerebral infarction in the pons and both hemispheres of the cerebellum associated with ovarian hyperstimulation syndrome has not been reported before but has devastating consequences for both mother and fetus. Strokes in patients with OHSS must be timely prevented, promptly diagnosed, and treated to avoid high morbidity and mortality associated with it.

RESUMO

We aimed to provide a clinical case presentation of aseptic meningitis as an extrahepatic manifestation of hepatitis C. A 28-year-old lady has been admitted to the Regional Clinical Center of Neurosurgery and Neurology, Uzhhorod City, Ukraine, with mild meningeal signs and symptoms upon admission. Complex neurological, clinical, laboratory, and imaging examination was performed within 24 hours of admission. Mononuclear pleocytosis of the cerebrospinal fluid and positive express test on HCV were discovered. The patient was treated and showed full recovery. Specific neurological features of aseptic meningitis as an extrahepatic manifestation of hepatitis C in a young white adult were reported, described, and analyzed.

RESUMO

OBJECTIVE: The aim: To analyse data from recent studies, dedicated to the use of non-steroidal anti-inflammatory drugs (NSAIDs); to evaluate the best clinical practice in the use of NSAIDs in order to prevent side effects (SEs) in different clinical scenarios; to optimise treatment of patients at risk of NSAIDs-related SEs. PATIENTS AND METHODS: Materials and methods: A comprehensive bibliographic search was performed using the keywords "NSAIDs", "NSAID gastropathy", "NSAID enteropathy", "complications of NSAID therapy", "cardiovascular disease", "cardiovascular risk" in the PubMed, Web of Science, Cochrane Library, Google Academy databases. CONCLUSION: Conclusions: NSAID-induced gastrointestinal lesions are а relevant problem of internal medicine, this is due to the fact that the pathogenic mechanisms of this process are still unclear. All the gastrointestinal tract (GIT) related risk factors(RFs) for gastro- and enterocolonopathies associated with the use of NSAIDs should be taken into consideration by physicians of all specialties. The examination and diagnostic of the GIT should be performed regularly to prevent complications. Uncontrolled, long-lasting, unprescribed NSAID usage should draw the attention of doctors, especially in patients with comorbid states.

RESUMO

We aim to report a COVID-19-related case of acute myelitis that has not been associated with any other viral infections. A 23-year-old student was admitted to the hospital within a month from the time of loss of smell and taste with features of acute-onset non-compressive myelitis with paresthesia on both sides from the Th9 level. Complex neurological, clinical, laboratory, and neuroimaging examination was performed within 24 hours of admission. MRI of the spine showed a segment of increased T2 signal in the center of the spinal cord at Th11-Th12. Elevated protein level and lymphocytic pleocytosis were detected in the cerebrospinal fluid. A serologic blood test for SARS-CoV-2 showed recent infection. PCR for other viral infections was negative. The patient was treated with injectable steroids and showed full recovery. Specific neurological features of acute myelitis associated with COVID-19 were reported, described, and analyzed. Patient was treated and recovered.

RESUMO

OBJECTIVE: The aim: To investigate the transcolonoscopic pH-metry and calprotectin in patients with ulcerative colitis. PATIENTS AND METHODS: Materials and methods: the research included 110 patients both male and female between the ages of 18 to 75 years old, who were treated for UC of medium and severe activity, in active phase. All patients were divided into 3 groups. The first group received standard therapy (ST; n=50), the second group received adalimumab (ADA; n=32), and the third group was treated with tofacitinib (TOF; n=28). The control group consisted of healthy individuals between the ages of 18 and 65 years old. RESULTS: Results: UC patients had lower pH levels in all sections of the large intestine, compared to the control group (р<0,05). Calprotectin level is a better predictor of the course of the disease. CONCLUSION: Conclusions:Tofacitinib, compared to adalimumab and budesonide, has better influence on clinical, endoscopic and laboratory parameters of UC.
